Notice of Privacy Practices

Health information shared during booking, intake, screening and treatment is treated as confidential. It is used to provide and coordinate care, to obtain clearance from a licensed medical provider where required, and to handle payment.

It is not sold and not used for advertising. It is shared only with the medical director and licensed providers involved in care, with a pharmacy where a prescription is required, and where the law requires disclosure.

Clients may request access to their records, request a correction, and request a restriction on how information is shared. Questions may be directed to Sunshine Mobile Wellness by email.
